The application code wouldn't, it would pass an identity down to the 
hosting runtime. I'm not sure yet the right way to deal with that at that 
level; we could have advisory information about when an existing proxy 
might time out, etc, or we could just check for the existence of one and 
handle a failing/expired one via the fault handlers.
