compiling issue with version 1.1.0 and C++ compiler

Dear parallel netcdf team:

There is a problem in pnetcdf.h in the release 1.1.0. -- The "#endif" on the
line 2132 should move to the end of the file for C++ compiler to be happy to
compile pnetcdf.h included files. Also, the line 261 should be removed where
put #include <mpi.h> inside extern "C" {} for C++ compiler. Thanks.
