Thank you for your prompt response and your advices.
To answer your questions, EFISPEC3D reads ExodusII mesh file generated by CUBIT. Boundary conditions are either free surface (made of QUAD) or absorbing boundaries (made of QUAD). But unfortunately, the NetCDF API inside CUBIT fail to generate a mesh of 200+ millions of hexa. The mesh is very basic: only a cubic domain meshed by millions of hexa. So I was looking for an open-source code (CUBIT is not) to generate such a mesh...

CGM alone would not be enough to generate a mesh
CGM is mostly a wrapper around other geometry kernels, ACIS or OCC/OCE based.
To generate a mesh you would need the SIGMA stack.

I am not familiar with what format you need your mesh for EFISPEC3D; I assume it is similar to NEK5000 solver, which is a spectral element method too. (so Nek needs a hexahedral mesh)
And what is the input geometry format? Terrain data?
We have some capabilities developed for ice sheet modeling. At the core, they do need a quadrilateral mesher for surfaces, which are then extruded/swept to form hexahedral elements. Also, it is assumed that the domain of interest does not have internal caves/tunnels, so the sweeping in vertical direction is trivial.
I am not sure how these would help your project.
