[AG-TECH] can't create institutional venue

I completed the form at

    http://www-unix.mcs.anl.gov/fl/research/accessgrid/venues/ivs.html

but only get the error response

    Error requesting new venue.
    Please resubmit your request.

I notice in the ag-tech archives that this problem was also reported back on 
Jan. 10. Is there another procedure for requesting an institutional venue?
