The multi input capture card most widely used in Australia is:

A Linux driver is built into the kernel since 2.4.22 & 2.6.0-test3,
with patches for some previous kernels available at:

I've done several installations with these cards, all trouble-free.
There are 2 models, IVC-200 & IVC-200G; we've always used the plain
IVC-200, without the GPIO module of the IVC-200G version.
