Jon,

Do you have auth.defaults file in ~/.ssh/ set? It has the following
structure:

<hostname>.type=key
<hostname>.username=<uname>
<hostname>.key=/path/to/key
<hostname>.passphrase=<passphrase>

file perm should be 600
