Well, it is difficult to get everything right. Here is me doing making a
reference tetrahedron:

I use CreateFormDAG() only to get the topology right, and then call
DMInterpolate() to get the edges/faces/orientations correct.
I can debug this code if you want, or you can use this method. Either way.
