[petsc-dev] manual build errors

Hi

I have just tried building the manual on the head of
petsc-dev/src/docs/tex/manual and found a few latex errors. Attached is
a patch which mostly just puts an escape before "_". However, in some
places it might be more appropriate to use \trl{}, so it needs checking.
