This looks like one of the bugs Dave fixed for mpich2-1.2.1
(https://trac.mcs.anl.gov/projects/mpich2/ticket/902). Are you not using
mpich2-1.2.1?

Can you try the following:

% mpiexec.hydra -n 2 -f machinefile hostname
