[MPICH] "Using MPI-2" authors implementation of Global Arrays

In reading the text "Using MPI-2", pp186-187, the authors talk about their implementation of functionality contained within the Global Arrays library. Looking at the book's web site, I am unable to find the entire code for the author's library. A web search found the original Global Array's library but not the sub-set written by the book's authors. Does anyone know from where the code for that sub-set library can be downloaded?
