itaps-parallel Phone conference and updated document

Reminder:  The parallel interface subcommittee will meet on Dec 6 at 11am
PST.  An updated Combined Document is attached.  We'll start the phone
conference with the "Not-Yet-Discussed, Unresolved Questions" section.

Details of the updates in the document:
-  Updated the background and terminology section to reflect recent mesh
instance discussions.  Added assumption that a partition covers at most one
mesh instance.

-  Added an EntitySet Functionality section, based on the results of our
mesh instance discussion.  If multiple meshes are stored in a single mesh
instance, an application needs to be able to perform global (with respect to
a partition) operations on members of an entity set.

-  Summarized questions resolved at the Nov. 28 phone conference.   See the
"comments and resolved issues" section.  Updated various parts of the
document to reflect the decisions we made.

-  Added a "Discussed but unresolved question" section that mostly outlines
our Nov. 15 phone conference.

-  Moved questions we have not yet discussed to the "Not-yet-discussed"
section.

-  Added entity functionality for obtaining the number of copies of an
entity in a partition and the remote handles of copies of entities in a
partition.  (See entity functionality section.)

-  Removed many references to MPI from the text (as requested by Mark
Miller).  The initial implementation is still MPI based.

-  Tried to make terminology more consistent as requested by Mark Miller.
